The practice is located at 163 Stratton Road. Stratton Road runs north to south from Rutland High School to the Rutland Regional Medical Center, the practice is approximately halfway between these two landmarks on the western side of Stratton Road.
Previously owned by Dr. Earl Olsen, our practice today is now ran by Drs. Tyler and Elizabeth Carmack. We offer a wide variety of services including Preventive Dentistry (Cleanings, Exams, and X-rays), Crown & Bridge Work, Root Canal Therapy, Cosmetic Dentistry, and Advanced Teeth Whitening treatments. The practice hours are Monday, Tuesday, and Thursday 8:00 a.m.-5: 00 p.m. and Wednesday 8:00 a.m.-6: 00 p.m. The office is closed daily from 1:00 p.m.-2:00 p.m. We welcome new patients and emergencies.
Please plan on coming 10-15 minutes early for your initial appointment as you will need to complete a Medical History Form and review the practice’s Health Information Privacy Practices. If you had imagining done within the last two years, please contact your previous dentist’s office to forward them to us.
Please notify the office PRIOR to your appointment if you have ever had to premedicate with an antibiotic prior to a dental visit or if you have a history of a heart murmur, mitral valve prolapse or have had a joint replacement as you may have to be premedicated prior to dental work.
It is important that you make every effort to attend all scheduled appointments. This is a very reliable, conscientious office that operates on an efficient schedule. Short-term cancellations and failed appointments cause openings that could be used for other patients, but might not be able to fill on short notice. We make every effort to have our patients start and finish their appointments on time.
It is our office policy that payment is expected, in full, at the time of your service for routine dental care, including exam, prophylaxis, x-rays, and fillings. Major dental work, including crowns, root canal therapy, bridges, and dentures require one-half payment at the first visit and the balance paid in full prior to the last visit. If insurance is involved, estimated co-payment is required at each visit. Once a treatment plan has been reviewed with you, the cost for services or estimated co-pays can be given to you prior to beginning any work. If you have insurance, please inquire about our insurance information sheet as dental insurance is typically quite different from medical insurance. You should also obtain a copy of your dental insurance plan and familiarize yourself with your dental benefits.
